Rejection can be classified by several clinical phenotypes. Of these phenotypes, late mixed rejection carries the worst prognosis. Eliminating DSA improves outcomes in rejection, including late rejection. Long established DSA are the most difficult to eliminate. PI therapy is effective in treating rejection. Combination of PI-base therapies may offer substancial improvement in efficacy.
