Home delivery program - Enrolment Form

Use this form to enrol your patient into the Sanofi Home Delivery Program to receive Eloctate (Efmoroctocog alfa) or Alprolix (eftrenonacog alfa) infusions at home.

The Sanofi Customer Service Team is responsible for the delivery of the product to your patient's home as authorised by your Haemophilia Treatment Centre (HTC). The Patient Home Deluvery Guide booklet will provide your patient with information on the Sanofi Home Delivery Program including the ordering process, your delivery schedule and frequently asked questions.
