Complications are generally categorized as microvascular, including retinopathy, nephropathy and neuropathy, and macrovascular, including coronary artery disease, stroke and peripheral arterial disease. Both categories result from prolonged exposure to elevated blood glucose affecting blood vessels of different sizes. Preventing or delaying these complications is a central goal of guideline-based diabetes management.
General glycemic targets typically aim for a glycated hemoglobin level associated with reduced complication risk, though specific targets are individualized based on patient age, comorbidities and hypoglycemia risk. Less stringent targets are often appropriate for older patients or those with significant comorbidities. Healthcare professionals should set individualized targets in line with current guideline recommendations rather than applying a single target universally.
Early diagnosis and prompt initiation of guideline-based management have been associated with reduced long-term risk of both microvascular and macrovascular complications, reflecting the cumulative effect of glycemic exposure over time. This is sometimes referred to as a legacy effect, where early control provides lasting benefit even if control later becomes more variable. This underscores the clinical importance of timely diagnosis and sustained management from the outset.
