L'hypertension, le diabète et la maladie rénale chronique partagent des mécanismes physiopathologiques qui s'aggravent mutuellement. Cette association accélère la progression de l'atteinte rénale et augmente significativement le risque d'événements cardiovasculaires. Une prise en charge globale et précoce est donc indispensable pour ralentir cette évolution. Les recommandations insistent sur un contrôle simultané de la pression artérielle et de l'équilibre glycémique.
Certaines classes antihypertensives offrent un effet néphroprotecteur particulièrement pertinent chez le patient diabétique avec atteinte rénale. Le choix thérapeutique doit également tenir compte de la fonction rénale résiduelle et du risque d'hyperkaliémie. Une adaptation posologique est souvent nécessaire en fonction du degré d'insuffisance rénale. Le suivi biologique régulier permet de sécuriser l'utilisation de ces traitements.
Une surveillance régulière de la fonction rénale, de la kaliémie et de l'équilibre glycémique est recommandée chez ces patients. Le contrôle de la pression artérielle doit être réalisé fréquemment pour ajuster le traitement si nécessaire. Une évaluation périodique de l'albuminurie permet de suivre la progression de l'atteinte rénale. Cette surveillance rapprochée s'inscrit dans une prise en charge multidisciplinaire du patient.
