Rosuvastatin exerts its effect through a mevalonic acid-like moiety that competitively inhibits HMG-CoA reductase. By reducing the conversion of HMG-CoA to mevalonate, rosuvastatin inhibits the early and rate-limiting step in cholesterol biosynthesis. The primary site of action is the liver, the target organ for cholesterol-lowering effects. Rosuvastatin increases the number of hepatic low density lipoprotein (LDL) receptors on the cell-surface, enhancing uptake and catabolism of LDL and inhibits the hepatic synthesis of very low density lipoprotein (VLDL), thereby reducing the total number of VLDL and LDL particles.
